Request Great Start Compensation Support Payment Program Supplier Verification Form

This request form is for licensed child care centers, licensed family child care providers, and certified child care centers who participate in the Great Start Compensation Support Payment Program and are interested in receiving their payments via direct deposit. Completing this request will send you an email to complete a verification form that will link your State of Minnesota Supplier ID to your future grant applications in order to be paid via direct deposit.

IMPORTANT: Before completing this form you should have already established a Supplier ID/Vendor ID on the State of Minnesota Supplier Portal. If you have not yet applied for or received a Supplier ID/Vendor ID, please do that before completing this form.
