Please use this form to contact the Disability Services Division for general questions and technical assistance related to home and community services and supports. For technical issues related to MMIS entry of service agreements, screening documents, or Home care PCA, HHA, SNV and HCN change, follow the steps in the “Contact the SASD Support Team” section on the CBSM – Service Agreement and Screening Document (SASD) Support Team page (formerly known as the Resource Center).

Role identification:

To report vulnerable adult abuse, neglect or financial exploitation, call Minnesota Adult Abuse Reporting Center (MAARC) at 844-880-1574.

If you are not able to pay your premium for Medical Assistance for Employed Persons with Disabilities (MA-EPD), you can fill out the MA-EPD Good Cause Request form, DHS-6939. It is the way you tell DHS why you do not have enough money to pay. If you have difficulty completing the form, review the instructions or call Disability Hub MN at 866-333-2466. The hub can help you submit the form.

Once you submit the form, it will come directly to DHS staff. We will review it and contact you with our decision.

In Minnesota, people with autism and related conditions can receive services and supports. The program is called the Early Intensive Developmental and Behavioral Intervention (EIDBI) benefit. It is for people who:

  • Are younger than age 21.
  • Are on Medical Assistance (MA), MinnesotaCare, Minnesota Tax Equity and Fiscal Responsibility Act (TEFRA) or other qualifying health care programs.
  • Have autism spectrum disorder (ASD) or a related condition.

For HHAX system users, check your provider portal to verify all NPI/UMPIs are listed, and an office is set up for each.

Exceptions are available to meet extraordinary costs incurred by providers when the RMS rate does not meet the extraordinary needs of some people who receive services.
